Initial Board Certification by ExamCertification
Eligibility (effective January 2008):
Five
hundred (500) hours of life care planning experience within 2 years of application
OR
A minimum of sixty (60) hours of continuing education units or its equivalent in contact hours specific to Life Care Planning from a University or Certificate Course.
Checklist for Application Submission:
1.
Completed application form and exam fee.

2.
Copy of current RN license or computer generated
document from the board of nursing demonstrating active license without restrictions.
3.
Candidates resume or curriculum vitae.
4.
Employer(s) letters demonstrating case management
hours and/or related nursing hours (4,000).
Continuum of care hours include related fields of nursing that involve the nursing
process and care needs assessments such as rehabilitation nursing and discharge
planning.

Proof of Life Care Planning Experience:

Life Care Planning hours need to be verified
on the application by the attachment of a letter from a supervisor/employer (minimum
of 500 hours). Medicare set aside training/experience
is approved for a maximum of 100 hours towards partial fulfillment of the 500 hours
requirement. Supervision by a Life
Care Planner in private practice holding certification who can verify 500 hours
of internship is also acceptable.
7.
Course outlines and certificates demonstrating
hours. This would include course outlines,
transcripts, verification of hours, continuing education units.
8.
Completed Life Care Plan with identifiable information
removed.
Registered Nurses that meet the eligibility requirements
and pass the certification exam may use the designation of CNLCP.
Initial certification is valid for 5 years.
Life Care Planning Program Review Criteria:
The certification board does not provide continuing education units. Using the criteria below the certification board will provide verification that a formal Life Care Planning program meets the minimum education necessary to sit for the exam.
Criteria for Appropriate Education Content:
1.
Proof
of documented contact hours or CEU's from a formal life care planning program.

Accredited
University
3.
University
programs and/or certification programs must provide educational material consistent
with the content outline found in the handbook.
This outline includes information on evaluating the care needs of various
catastrophic/chronic injuries/illnesses as well as identifying items to improve
and/or maintain maximum function/care:
A. Life Care Planning
B. Spinal Cord Injuries
C. Burns and Amputations
D. Neonatal and Pediatric Care
E. Chronic Pain
F.
Traumatic Brain Injuries
Any program that is in question should be submitted
to the Certification Board 90 days in advance advance of any examination date so
that the above criteria can be verified.
